Consumer Periodics (035.06)
- Changed title from 'State Periodics' to 'Consumer Periodics'
- Changed transaction definition from 'State Periodics are collected at intake, and reported at least every three months, or on change' to 'Consumer Periodics are collected at intake and on change.'
- Changed field description of 'modate' from 'Month of Periodic' to 'Effective Date'
- Changed definition of field 'modate' from 'The year and month of the periodic information as reported by the clinician' to 'The date that the initial Periodics were collected at intake or the date that one of the data elements in the Periodics changed.'
- Changed Actions Allowed from 'NA' to 'LA' on the following fields:
- Grade Level
- Priority Code
- Made changes to Living Situation code table to align with State Data Dictionary
- The following codes are retired:
- 10 - Private Residence Without Support
- 20 - Private Residence Receiving Support
- 40 - 24-Hour Residential Care
- The following codes are new:
- 11 - Permanent Housing - Unassisted
- 12 - Permanent Housing - Assisted
- 21 - Temporary Housing - Unassisted
- 22 - Temporary Housing - Assisted
- 23 - Temporary Housing - Dependent
- 24 - Transitional Housing
- 31 - Adult Family Home
- 32 - Residential Care
- 42 - Crisis Residence
- The following code definitions were adjusted to align with State Data Dictionary
- 30 - Foster Home
- 50 - Institutional Setting
- 60 - Jail/Juvenile Correction Facility
- 70 - Homeless/Sheltered
- The following codes are retired:
Program Episode Identification
- Episode Type - Added items specific to SAMHSA-Required EBPs. These all use numeric codes and have 'SAMHSA EBP' at the beginning of the description. Only report these EBP episode types where the program/service is designed to meet fidelity and fidelity is monitored.
Service Disposition
- Added code 39 - Discharged to Corrections, Jail
Consumer Demographics
- Made changes to the Sexual Orientation code table to align with State Data Dictionary
- Retired code 2 - The person states they are gay, lesbian, bisexual, or transgendered.
- The following codes are new:
- 3 - The person states they are gay or lesbian
- 4 - The person states they are bisexual
- 5 - The person states they are questioning
- Modified code '9' by adding ', N/A' to the end of the definition.
